Tesamorelin (TH9507) is a synthetic 44-amino-acid GHRH analog stabilized by a trans-3-hexenoic-acid modification that resists DPP-4 cleavage. It is the only GHRH analog to receive FDA approval, originally for HIV-associated lipodystrophy. Reported research benefits: selective reduction of visceral adipose tissue (~15–18%) without loss of subcutaneous fat; decreased hepatic steatosis in NAFLD/MASLD imaging studies; restoration of the IGF-1 axis in age-related GH decline; improvements in triglycerides, ApoB, and adiponectin; support for lean-mass preservation during caloric restriction; and measurable improvements in executive function and verbal memory in adults with mild cognitive impairment.
